Documents For Folders
After Your Adoption Info Sheet - This should be included in every foster cat's folder.
Expanded Medical Info Sheet - This sheet should be present and filled out in every cat's medical folder at the start of fostering the animal. Keep it updated and in the folder.
Foster Folder Check list - When taking in a new foster cat, fosters should make sure that the documents listed on this checklist are all in the cat's folder and properly filled out. To complete the checklist after bringing the foster cat home, everything on the list is available on this webpage except for the Vet Info Sheet.
Other Forms
Virtual Home Check Form - Prior to the home check, save a copy of this document with the adopter’s name and preferably the cat's name (if known) in the file name, i.e.: “Home Check - Jane Smith adopting Mittens” and email it to the ASHA email after completed.
Petco Featured Feline Cage Info Sheet - Needed per cat when at the store. To be printed and placed inside the cage's sheet cover.
Veterinary Visit Forms
Padonia Vet Appointment Form - Need to complete one per cat per visit. Fecal samples must be double bagged, labeled and attached to the form.

Spay Now Laurel Surgery Form - One form per animal. This form is for an indoor cat/kitten getting spayed/neutered, rabies, FVRCP and dewormer. Foster must fill in their name and phone number so the staff know who will be doing pick up. Also, foster must fill out the animals info. Spay Now has our card on file.
